EMPLOYEE BENEFITS

Cost Sharing and Premiums

The company contributes a portion of the medical insurance plans and 100% of the

basic life insurance. Employee contributions for premiums or specific benefits will

be deducted from their bi-weekly pay. Current premium rates are communicated

during enrollment periods and are available from the HR Department.

Terminating Benefits

Upon termination of employment, benefit coverage will cease according to the

terms of each individual plan and applicable laws (e.g., COBRA). Information

regarding the continuation of coverage options will be provided by the HR

Department.

Confidentiality

All employee benefit information, including health data and enrollment choices, is

considered confidential and will be managed with the utmost discretion and in

compliance with privacy regulations (e.g., HIPAA). Access is restricted to authorized

team members only.

Discrepancy Resolution

Should an employee identify a discrepancy or have a question regarding their

benefits, they should:

Contact the HR Department directly to report the issue.

The HR Department will investigate the reported concern promptly and

communicate the findings and resolution steps, if any, to the employee.
