EMPLOYEE BENEFITS

Health Insurance:

Medical: Comprehensive medical coverage options, including an HSA-

qualified health plan. While the company offers an HSA-qualified plan,

employees are responsible for establishing and managing their Health

Savings Account (HSA). with a bank of their choice.

Dental: Voluntary coverage for preventative and restorative dental care

(100% employee paid).

Vision: Voluntary coverage for eye exams, glasses, and contact lenses (100%

employee paid).

Retirement Plan:

401(k) Plan: A retirement savings plan that allows employees to contribute

pretax or Roth dollars, with a discretionary employer contribution.

Paid Time Off (PTO): A combined bank of time for vacation, personal

appointments, and other personal needs. (Refer to the PTO policy for detailed

accrual and usage guidelines).

Sick Leave: Accrued time off for illness or medical appointments. (Refer to the

separate Additional PTO policy for detailed accrual and usage guidelines).

Company Holidays: Paid days off for recognized holidays throughout the year.

Life and Voluntary Insurance Coverage:

Basic Life Insurance: $10,000 company-paid life insurance coverage for

full-time employees only (100% company paid).

Voluntary Supplemental Life Insurance: Option for employees to

purchase additional life insurance coverage for themselves, their spouse,

and children.

Voluntary Short-Term Disability (STD): Income protection for

temporary periods of disability.

Voluntary Accident Insurance: Coverage for expenses resulting from

accidental injuries.

Voluntary Critical Illness Insurance: Provides a lump-sum payment

upon diagnosis of a covered critical illness. Employees can also elect to

purchase coverage for their spouse and children.

Voluntary Hospital Indemnity Insurance: Provides a daily benefit for

hospital confinement.
