Responsibilities
Employee Responsibilities:
Understand and comply with the terms of this policy.
Provide timely and accurate notice for leave requests.
Submit all required documentation promptly.
Communicate regularly with Human Resources regarding their leave status
and anticipated return to work.
Adhere to any conditions or restrictions placed on their leave.
Manager Responsibilities:
Notifying Human Resources: Managers who receive a request for leave or
become aware of an employee's need for leave for any reason (including, but
not limited to, medical conditions, family care needs, military service, or
domestic violence/sexual assault situations) must immediately notify
Human Resources. Even if the employee states they do not wish to take a
formal leave or believe they are ineligible, the manager must still notify HR
so that HR can determine eligibility and provide the employee with all
necessary information and rights.
Advising Employees: Managers should advise employees who inquire about
or indicate a need for leave to contact Human Resources directly for
detailed information regarding their eligibility, rights, and responsibilities
under this policy and applicable laws. Managers should not provide
definitive advice on leave eligibility or duration.
Human Resources Responsibilities:
Administer this policy consistently and in compliance with all applicable
federal and state laws.
Provide employees with necessary information and forms related to leave
requests.
Maintain confidentiality of employee medical and personal information
related to leave.
Communicate decisions regarding leave requests in a timely manner.
Policy Administration
The Human Resources Department will administer this policy. Any questions
regarding this policy or specific leave situations should be directed to Human
Resources.
